Pre-register at www.eAYSO.org, print and sign 2 copies of the registration form, mail both copies of the forms, payment and a copy of the player’s birth certificate to
(players will not be placed on teams/waiting list until all forms have been received and processed by the registrar) :

Registering 1 Player in Family: $100
Registering 2 Players in Family: $200
Registering 3 or more Players in Family: $250
AYSO Region 498 defines 'Family' as players within the same household and registered at the same time/event.

HOW TO REGISTER YOUR CHILD:
Follow the instructions onscreen to fill out and print 3 player forms.
For any problems "DIRECTLY" related to the e-AYSO pre-registration process, please call the National e-AYSO help desk at (866) 588-2976. Or you may email a question to eaysosupport@ayso.org. Our region has no jurisdiction over Eayso. (NOTE: pop up blockers must be turned off in order to print forms.)
Returning players: please make sure all phone numbers and email addresses are current since this is how your coach will contact you.
New Players: make sure that you have chosen AYSO Region 498, Madison, AL and fill in the experience information.
Make sure all three forms are signed. The Official AYSO Player Form has a medical waiver written into it that AYSO National requires us to have on hand for insurance purposes. The coach must have an original copy in his hands before your child can play or practice. The registrar must keep a copy on hand for 7 years. The third copy will be your receipt and is optional. If your child has never played a sport for The City of Madison, please bring a copy of the birth certificate for us to put on file.
Your child will not be placed until we have the following: a copy of the birth certificate on file, 2 signed copies of the player form and payment for the correct amount. Bring 3 copies of the form to one of the Walk-In registrations listed above along with your payment. Cash or checks, please. We can not accept credit or debit cards.
PLEASE VOLUNTEER!!
AYSO is a volunteer-driven organization. Without people willing to give their time to coach and to referee, there would be no soccer program.
WE HATE TURNING KIDS AWAY, but we are limited to the number of kids we can accommodate by the number of coaches and referees we have. AYSO National has set the maximum number of children that can be on a team. IF you volunteer, your children are guaranteed a place on a team as long as you volunteer before late registration. After that we can guarantee a place only for the child you would be coaching. Any other child would be placed at the top of the appropriate waiting list. Please make sure your child’s Player Form indicates that you are a volunteer and what you are volunteering to do (coach, referee etc).
If you are going to volunteer, please do so as early as possible since we need to have a good idea of the number of spots available for the kids. Due to identification requirements, we are no longer able to accept Volunteer forms by mail. An exception can be made if you turned in a Volunteer Form during the previous year. Please contact our Child & Volunteer Protection Advocate at this email address for permission to submit your Volunteer Form by mail:
All new volunteers must come to a walk in registration to volunteer. We have fill-in forms available, but it is suggested that you go to www.EAYSO.org and fill out your volunteer form. The benefit to filling out this form online is that the complete social security number and driver’s license number are not printed out on your volunteer form. AYSO National must have these numbers for the background check, but our local volunteers do not need them. This protects your privacy. MISC INFO:
We try to honor carpooling requests, but cannot guarantee them. We can not honor requests for certain coaches at all since this practice would lend itself to team stacking.
Practices will start around the beginning of August. The first game is generally in the middle of August. The Coaches Meeting (where the coaches receive the player forms for their team) is generally in late July. You will not hear from a coach until after this date. If you have not heard from anyone by Aug. 1st, please contact the appropriate Division Coordinator. Email addresses are listed on our website under the Board & Staff link at www.ayso498.org
